On a personal level, the clinic I attended had a 'policy' whereby

if a patient had attained a Level 2, 3, or 4 Status; a urine screen

could be requested randomly.

 

On the other hand, if a patient was a Level One, ( a beginner 

or new to Methadone Maintenance Therapy) they would have
